We are Care Connection, your non-medical home care connection to the Stratford Visiting Nurse Association (SVNA). Since 1919 the Stratford VNA has been a trusted source of home health care throughout Fairfield and New Haven Counties.

Care Connection by The Stratford VNA is an independent, local, nonprofit, home care organization serving most towns in Fairfield and New Haven Counties. They are most decidedly not a franchise of a big, national, for-profit company like many of the home care providers. All of their management and caregivers live here, right here, in your community.

Family Members Kept Informed
Through our interactive Family Room, an online web portal, family members have real-time transparency into the care of their loved ones. Care activities are carefully recorded. You can keep live track of caregiver status updates, coordinate and schedule events with other family members and more.
